    Benchmarking multi-criteria evaluation methodology's application for the definition of benchmarks in a negotiation-type public-private partnership. A case of study: The integrated action programmes of the Lazio Region

    The growing scarcity of public financial in Italy, in opposition of the more significant problems of degradation of many urban areas, prompted the Legislature to standardise new processes of settlement transformation based on negotiation-type public-private partnerships (PPPN). However, these standards have not provided for benchmarks referring to the contents of partnerships or assessment procedures aimed at assessing the initiatives undertaken with respect to public utility objectives. This has often led to redevelopment initiatives geared more towards the satisfaction of private rather than public interests. The proposed methodology, structured on the integration of a benchmarking process with multi-criteria evaluation techniques known as benchmarking multi-criteria evaluation (BME) enables the definition of benchmarks through a participatory process of the different stakeholders involved in a PPPN to which the BME is applied. In order to verify the applicability of the proposed procedure, it has been applied to a type of PPPN: the integrated action programmes (PII) in the Lazio Region. The benchmarks can be used by Lazio’s administrators both for renewing the planning of the PII concerned and for verifying the quality of the initiatives within the same PPPN proces

    IL2 treatment for cancer: from biology to gene therapy.

    In this review we shall discuss the biological rationale and the clinical findings obtained using Interleukin 2 (IL2)-based immunotherapy in the management of cancer patients. Objective and long-lived clinical responses have been documented in a proportion of cases, particularly renal cell carcinoma, melanoma and acute myeloid leukaemia. Though encouraging, the clinical use of IL2 has so far been limited by toxicity, as well as by the heterogeneous and unpredictable responses and by the lack of specific anti-tumour effect. These considerations have led to the belief that more sophisticated technologies aimed at introducing the IL2 gene into the neoplastic cells may potentially overcome some of the limitations coupled to the in vivo infusion of high doses of IL2. The data accumulated in animal models and, more recently, also with human tumour cells indicate that the IL2 gene may be successfully inserted into neoplastic cells. The constitutive secretion of IL2 by the tumour cells leads to a reduced or abrogated tumorigenicity in several different tumour models. The evidence that in some experimental tumours the transduction of the IL2 gene into the neoplastic cells may elicit a specific cytotoxic response and confer anti-tumour memory, suggests that vaccination protocols based on this innovative strategy may represent a potential new tool in the management of cancer patients

    High concordance between trained nurses and gastroenterologists in evaluating recordings of small bowel video capsule endoscopy (VCE)

    Background & Aims: The video capsule endoscopy (VCE) is an accurate and validated tool to investigate the entire small bowel mucosa, but VCE recordings interpretation by the gastroenterologist is time-consuming. A pre-reading of VCE recordings by an expert nurse could be accurate and cost saving. We assessed the concordance between nurses and gastroenterologists in detecting lesions on VCE examinations. Methods: This was a prospective study enrolling consecutive patients who had undergone VCE in clinical practice. Two trained nurses and two expert gastroenterologists participated in the study. At VCE pre-reading the nurses selected any abnormalities, saved them as “thumbnails” and classified the detected lesions as a vascular abnormality, ulcerative lesion, polyp, tumor mass, and unclassified lesion. Then, the gastroenterologist evaluated and interpreted the selected lesions and, successively, reviewed the entire video for potential missed lesions. The time for VCE evaluation was recorded. Results: A total of 95 VCE procedures performed on consecutive patients (M/F: 47/48; mean age: 63 ± 12 years, range: 27−86 years) were evaluated. Overall, the nurses detected at least one lesion in 54 (56.8%) patients. There was total agreement between nurses and gastroenterologists, no missing lesions being discovered at a second look of the entire VCE recording by the physician. The pre-reading procedure by nurse allowed a time reduction of medical evaluation from 49 (33-69) to 10 (8-16) minutes (difference:-79.6%). Conclusions: Our data suggest that trained nurses can accurately identify and select relevant lesions in thumbnails that subsequently were faster reviewed by the gastroenterologist for a final diagnosis. This could significantly reduce the cost of VCE procedure

    A Bayesian analysis of neutron spin echo data on polymer coated gold nanoparticles in aqueous solutions

    We present a neutron spin echo study (NSE) of the nanosecond dynamics of polyethylene glycol (PEG) functionalised nanosized gold particles dissolved in D2_2O at two temperatures and two different PEG molecular weights. The analysis of the NSE data was performed by applying a Bayesian approach to the description of time correlation function decays in terms of exponential terms, recently proved to be theoretically rigorous. This approach, which addresses in a direct way the fundamental issue of model choice in any dynamical analysis, provides here a guide to the most statistically supported way to follow the decay of the Intermediate Scattering Functions I(Q, t) by basing on statistical grounds the choice of the number of terms required for the description of the nanosecond dynamics of the studied systems. Then, the presented analysis avoids from the start resorting to a pre-selected framework and can be considered as model free. By comparing the results of PEG coated nanoparticles with those obtained in PEG2000 solutions, we were able to disentangle the translational diffusion of the nanoparticles from the internal dynamics of the polymer grafted to them, and to show that the polymer corona relaxation follows a pure exponential decay in agreement with the behavior predicted by coarse grained molecular dynamics simulations and theoretical models. This methodology has one further advantage: in the presence of a complex dynamical scenario I(Q,t) is often described in terms of the Kohlrausch-Williams-Watts function that can implicitly represent a distribution of relaxation times. By choosing to describe the I(Q,t) as a sum of exponential functions and with the support of the Bayesian approach, we can explicitly determine when a finer-structure analysis of the dynamical complexity of the system exists according to the available data without the risk of overparametrisation

    Effective coping with cyberbullying in boys and girls: the mediating role of self-awareness, responsible decision-making, and social support

    There is sufficient evidence on the negative consequences of cyberbullying victimisation depending on the coping styles. Social support seeking is among the most effective strategies for coping with cybervictimisation, but it is scarcely used. The robust Maximum Likelihood (ML) method was used to test the potential mediating role of individual (self-awareness, and responsible decision-making) and contextual variables (self-perceived parental and peer support) in the relationship between cybervictimisation and social support seeking in boys and girls. This cross-sectional study collected data from 1,276 Spanish secondary school students (51.2% boys, 48.8% girls) aged 11-18 (M = 13.88, SD = 1.42). Structural equation modelling (SEM) results pointed out responsible decision-making and self-perceived parental support as relevant mediating factors for girls. By contrast, the model was not significant for boys. These findings highlight the importance of both individual and contextual variables in helping adolescents cope with cyberaggressions, considering gender differences

    In silico evaluation of a control system and algorithm for automated insulin infusion in the ICU setting

    <p>Abstract</p> <p>Background</p> <p>It is known that tight control of glucose in the Intensive Care Unit reduces morbidity and mortality not only in diabetic patients but also in those non-diabetics who become transiently hyperglycemic. Taking advantage of a recently marketed subcutaneous glucose sensor we designed an <it>Automatic Insulin Infusion System </it>(AIIS) for inpatient treatment, and tested its stability under simulated clinical conditions.</p> <p>Methods</p> <p>The system included: reference glucose, glucose sensor, insulin and glucose infusion controllers and emergency infusion logic. We carried out computer simulations using Matlab/Simulink<sup>®</sup>, in both common and worst-case conditions.</p> <p>Results</p> <p>The system was capable of controlling glucose levels without entering in a phase of catastrophic instability, even under severe simulated challenges. Care was taken to include in all simulations the 5-10 minute delay of the subcutaneous glucose signal when compared to the real-time serum glucose signal, a well-known characteristic of all subcutaneous glucose sensors.</p> <p>Conclusions</p> <p>When tested <it>in-Silico</it>, a commercially available subcutaneous glucose sensor allowed the stable functioning of a proportional-derivative Automatic Insulin Infusion System, which was able to maintain glucose within acceptable limits when using a well-established glucose response model simulating a patient. Testing of the system <it>in vivo </it>using animal models is now warranted.</p

    Deposits, composition and technological behavior of fluxes for ceramic tiles

    The ceramic tile industry is a big consumer of feldspathic rocks and further raw materials able to act as flux during firing. This demand is currently fed with the recourse to a wide range of sources of igneous, sedimentary, metamorphic and metasomatic origin. The main geological features of flux deposits actually exploited by the ceramic industry are reviewed. The chemical and mineralogical composition is described to have a general picture of raw fluxes as well as average data and compositional perimeter for deposits of different origin. Overall beneficiation routes are drawn to trace the compositional path from deposit to commercial flux. The technological behavior in tile-making is shortly overviewed and new experimental data are provided on flux fusibility (by hot-stage microscopy).
